Connect Project
Places on the Connect Program are funded by the NSW Department of Ageing, Disabilities and Home Care under the Disabilities program. Connect helps people with a disability live more independently in the community. At present we have funding for 5 Connect places.
What is Connect?
Connect helps you plan and organise services you need to live as independently as possible. This is a short term program, which should be completed within 8 weeks.
Connect tries to help you:
How does Connect do that?
One of our coordinators will meet with you and:

Help you work out your needs and goals

Give you information on what choices may be available to meet your individual needs, like where to get equipment

Give you information on what services are available so that you can make a choice

Help you work out what you need to do to meet your needs and achieve your goals

Help you access other services like TAFE, physiotherapy, home Care

Develop with you a support plan

Supporting you when you need to talk to other services or authorities, like the Department of Housing - Advocacy

Referrals to other services, like physiotherapy, occupational therapy, hydrotherapy, counselling

Adults under 65 years of age (or under 45 years if of Aboriginal or Torres Strait Island Background) who have a disability and who:

When we have a vacancy on the Connect program, we will ring you to make an appointment to visit you in your home.
