What is 'ComPacks'?
A short-term community service funded by the NSW Department of Health. After a stay in hospital, ComPacks helps you with tasks like personal care or housework as well as linking you into community services.
Who can go home with ComPacks?
People who are discharged from the Coffs Harbour Health Campus, including people who:

Cannot manage at home without support after leaving hospital

Need two or more community services to help them for a short period or maybe longer

Need help to plan and apply for ongoing community services
What will ComPacks help with?

The Community Care Options Coordinator can help you with:

Assessing your needs and setting goals

Arranging services

Checking the services are right

Making referrals and getting other community services

Linking you to services and support you need after Compacks have ended.
